CIFHS is committed to protecting your privacy when you visit our websites. We want to inform you about the information we collect when you visit our site and how we use it. Specifically, we use your information to help you complete a transaction, communicate back to you, update you on services and benefits, and personalize our websites. From time to time, we may also use your information to contact you or provide you with information that we believe will be of particular interest to you. CIFHS does not sell, rent, or trade any personal data visitors submit to our site to any third parties. The company respects the privacy of users visiting our site and complies with all applicable laws regarding the collection, use, and disclosure of personal information. Occasionally, we may use the directory information we have collected to send you information about products, services, updates, and other relevant information that we believe may be of interest to you. If you indicate that you do not wish to have this information used as a basis for further contact, we will respect your request. You can email admin@cifhs.org to request your name be removed from our lists or to change or modify your information. 

The data you provide will be held in strict confidence by CIFHS and used solely for the purposes of the company. We will not share your data with anyone else unless you permit us to do so or as required by law.

Our privacy policies apply only to official CIFHS websites. This site contains links to other sites that may indicate a special relationship with us. While we do not disclose personally identifiable information to those operating these linked sites, we are not responsible for the privacy practices of such other sites. You should read the privacy policies of each site you visit to determine what information that site may be collecting about you. 

SMS Terms & Conditions 

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information collected during the SMS opt-in process will not be shared with third parties, including affiliates or business partners. The opt-in is not transferable to another party involved in the process. Your use of SMS is subject to your carrier’s terms of use, and CIFHS is not responsible for any of those terms. 

You may agree to receive SMS messages and opt-in from CIFHS during phone calls, web forms, and other mediums. Our SMS messages include appointment scheduling, appointment reminders, post-visit instructions, lab notifications, and billing notifications.  

Here are some additional reminders:  

  • Message frequency may vary depending on your usage. 

  • Message and data rates may apply for any messages sent to you from us and for messages sent to us from you. If you have any questions about your text plan or data plan, it is best to contact your wireless provider. 

  • If you forget what words are supported, text “HELP” to (626) 966-1577. After you send the message “HELP” to us, we will respond with instructions on how to use our service and unsubscribe. 

  • Reply “STOP” to cancel at any time. We will send you a reply message to confirm that you have unsubscribed. After this, you will no longer receive messages from us. If you'd like to join again, sign up as you did the first time, and we’ll send you messages again.
